Provides skilled nursing care to patients in their home, under the supervision of the Director of Nursing. Provides service in accordance with CHESAPEAKE HOME HEALTH CARE, INC. philosophy, standards and polices. Provides skilled nursing care in the home as ordered on the Medical Plan of Treatment and Nursing Care Plan. Applies independent emergency measures to counteract adverse developments and notifies physician of patient?s condition. * Skilled in assessing and reporting findings in accurate, concise and professional manner. * Observes aseptic techniques as required. * Performs specific treatments and administers medications as ordered. * Educates patients and significant others regarding the patient?s illness, procedures, treatments, including self-care, prevention and health maintenance. * Assists patient and/or family in keeping home an environment safe. * Prepares meals for patient appropriately. * Keeps patients supplied with necessary materials. * Records information accurately, legible and concisely following Medical Plan of Treatment. * Keep supervisor and staff advised of changes in patient?s condition. * Document all changes in patient?s physical or psychosocial condition EDUCATION AND MANDATORY?QUALIFICATIONS: * Graduate of an accredited school of nursing * Possess a current RN License in the District of Columbia/Maryland?or compact state. * One-year pediatric experience within the last three years. * One-year of previous adult patient experience. * Current CPR * Basic life support and first aid training * Company Health Requirements * Initial Physical Exam (if providing direct patient care) * Initial Negative PPD/TB Skin Test * Annual TB/PPD testing * Chest X-Ray with Initial Tuberculosis Questionnaire * Annual Tuberculosis Surveillance Questionnaire if a Chest X-ray was provided initially. * Hepatitis B Vaccine (letter of acceptance of declination to be kept on file) * Subject to random drug screening, upon reasonable suspicion. * Pre-Employment Criminal Background Check with?satisfactory results * Maintains optimum health and well-being of the patient POSITION QUALIFICATIONS Competency Statement(s) * Accountability Ability to accept responsibility and account for his/her actions. * Accuracy Ability to perform work accurately and thoroughly. * Analytical Skills Ability to use thinking and reasoning to solve a problem. * Communication, Oral Ability to communicate effectively with others using the spoken word. * Communication, Written Ability to communicate in writing clearly and concisely. * Customer Oriented Ability to take care of the customers? needs while following company procedures. * Detail Oriented Ability to pay attention to the minute details of a project or task. * Diversity Oriented Ability to work effectively with people regardless of their age, gender, race, ethnicity, religion, or job type. * Organized Possessing the trait of being organized or following a systematic method of performing a task. * Relationship Building Ability to effectively build relationships with customers and co-workers.
